Paper Transcript Request Form
Policy: Payment is required with this online request. You may do so after submitting this form. Allow seven business days for processing. Requests will be processed as quickly as possible, in order of receipt. Your account with the seminary must be current, or your transcript will be withheld. Transcripts from other institutions cannot be duplicated; you must contact the institution directly.
Cost: The transcript fee is $10.00 per copy. There is no fee for this service for current students and recent graduates. May graduates may request transcripts free of charge through November after graduation. December graduates may request transcripts free of charge through June after graduation. All work completed at Louisville Presbyterian Theological Seminary is considered one transcript. Transcript requests must be accompanied by the fee. Online requests must be paid by credit card using our secure, online payment process following the submission of this form. You will be instructed how to do this after you complete this form and select "submit" below.
Printed Requests should be mailed to Louisville Seminary Registrar and must be accompanied by a check, made payable to Louisville Seminary.
